1.  Click HERE
(if your computer warns you that it's unsafe to proceed, ignore this and click on "Advanced.") 

Then click on the link that appears at the bottom of the page. 

This will direct you to the FORMED signup webpage.
 
Then, proceed to step # 2.

2.  If you have already set up your account you may simply click "sign in" and then enter your email address.  (If you have not set up an account, proceed to step 3. 

3. If you HAVE NOT set up your account, please find St. Mary Church by typing 44035 into the field. 
St. Agnes Church and St. Mary's Church will appear. 

4. Highlight St. Mary's Church and click on it.  St. Mary's Church will now appear in the field. 

5. Click NEXT

6. Enter your name and your email address and click "SIGNUP."

7.  Follow instructions from this point on.
